Note: This is a printable version in PDF format. To create the book:

• Print it (Before printing, make sure the boxes "shrink oversized pages to paper size" and "auto-rotate and center pages" are checked in the Print Menu. The command of Ctrl + P will bring up the screen allowing the selection of these boxes.)
• Alternately rotate the pages so they face head up/head down
• Copy it from one to two sided
• Fold and staple
