Check Out and Order Information

Procedure to Use 4-H Materials:

1. Complete the Teacher Enrollment Form
2. Complete the Order and Check-Out Form (see below)
3. We will work with you through ESD Courier or other methods to help delivery.

*Each person is responsible for checking the materials to make sure that all of the pieces are intact and returned. Please report any defects or problems you have with the material. Materials may be checked out for a two week time period. After two weeks materials must be returned or re-checked out!!!
