Medical Treatment Needed

If an incident results in an employee's need for medical treatment, both the employee and the employee's supervisor must complete the SAIF 801 Form.  If an employee is injured but does not intend to file a workers' compensation claim with SAIF Corporation, the employee still completes the SAIF 801 Form, but does not sign on the signature line.

Within 24 hours of the incident fax the SAIF 801 Form to Heidi Melton, OSU Human Resources. Fax: 541-737-0553.

In addition, an employee will need to obtain a completed medical release from his/her attending physician after every treatment for an on-the job- injury using an Employee Status Report Form.

  • Priority levels for Safety Inspection Correction:
    • Priority 1: Items are immediate safety hazards and need to be corrected within 48 hours, if possible.
    • Priority 2: Items should be corrected within five weeks.
    • Priority 3: Items are long-term fixes that should be either added to an ongoing work list or to a capital improvement list.

Risk Management

The Office of Risk Management (ORM) is committed to the stewardship of human, fiscal, and physical resources entrusted to OSU. ORM staff work with the OSU community to identify areas of potential risk and develop the most effective solutions.  Their goal is to minimize the risk of loss to OSU.  

Liability Waivers & Releases

Please use only the forms approved by the Office of Risk Management or the Office of General Counsel.  These are binding legal documents.

Property Loss or Usage

Report any loss to the Office of Risk Management immediately.

If the loss is due to theft, break in, or vandalism, also report it to law enforcement immediately. 

  • If it occurred on the University campus: contact the OSU Department of Public Safety at 541-737-3010
  • If it occurred elsewhere: contact the police department for that jurisdiction. You will need to get a copy of any police report.

Loan Agreements

  • Intra-department Property Loan Agreement (PDF)
    An employee may borrow property for off-campus university business, with authorization. This document summarizes the policy and should be completed for items on loan.
  • Personal Property Loan Agreement (PDF)
    When an employee chooses to use his or her own personal property to complete university business.  Complete this form and keept it on file.

Certificate of Insurance

To request a certificate, contact Procurement and Contract Services (PaCS). Certificate are issued by the DAS-RMD in Salem and can take up to seven business days to complete.
