Members:

All Gilliam County 4-H members must complete the member packet and return it to the Gilliam County Extension Office, PO Box, 707, Condon, OR  97823.

Enrollment Deadlines:

Returning 4-H Members – November 16, 2015
New 4-H Members – December 15, 2015

Leaders: 

All Gilliam County 4-H Leaders must complete the leader packet each year and complete a background check every 2 years. The leader must also complete a new leader training.

Please complete the Criminal History Check Disclosure Notice and Release Authorization and send to Oregon State University, Office of Human Resources, 122 Kerr Admin Bldg, Corvallis, OR  97331-2132. If you are an animal leader, please be sure and circle the "A" where the other circles are on the front of the form.

If you drive 4-H members, please be sure and check the "Extension Volunteer - driving" box on the Criminal History Check Disclosure Notice and Release Authorization and submit the following per instructions found on the form: Order Your Own Record from DMV There is a $1.50 fee.

Gilliam County 4-H Ambassador Application -

As a County Ambassador you will representing the Gilliam County 4-H program through the county.  Your duties will include working with a team to plan, coordinate and implement 4-H county events.  The County Ambassadors will also participate with the Gilliam County 4-H Social Media pages.  Applicants must be in 9th through 12th grade at the time of application.

Please fill out the Gilliam County 4-H Ambasador application and return it to the Gilliam County 4-H Office.
