Living Well with Chronic Conditions

Living Well with Chronic Conditions is a six-week workshop that provides tools for living a healthy life with chronic health conditions, including diabetes, arthritis, asthma and heart disease. Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with the emotions that chronic conditions may bring about.
Classes are fun and interactive. Participants share their successes and build a common source of support. The workshop builds confidence around managing health, staying active, and enjoying life.

  • Working with your health care team
  • Setting weekly goals
  •  Effective problem solving
  • Better communication
  • How to relax
  • Handling difficult emotions
  • Tips for eating well
  • Safe, easy exercise

Who Attends: People with different chronic conditions and their family, friends or caregivers.

What You Will Learn: Resources, skills, and action planning to give you a sense of greater control over life with a chronic condition.

Leaders: Two trained leaders.

Registration & Cost: $15 material fee, includes a resource book, is due at time of registration.
