4-H Enrollment 2015-2016

The 2015/16 4-H Year begins October 1.

STEP 1: FIND A CLUB-Leader Directories are available at the Extension Office. 
(Note: If you are new to 4-H and are looking for an active club to join,  first determine a 4-H project in your area of interest and then find a club involved in that project.)  

STEP 2: ENROLLMENT & ANNUAL FEE
Select, print, and fill out the Member Enrollment Form below. Then return the form and fee to the OSU Extension Office, SOREC, 569 Hanley Rd. Central Point, OR 97502) Payment options

AGE REQUIREMENT:  Jackson County has numerous clubs in the traditional 4-H program. Most clubs are project-oriented, for grades 4-12. Some are mixed project or community clubs. Youth must currently be at least **9 years old (Grade 4) by September 1, 2015, to enroll.
***Ages 5-8 youth must wait until the following year(s) but are still eligible to be in the 4-H Cloverbud program.

 Enrollment in 4-H clubs for 2015-16 begins in October 1, 2015.

*To participate in the County Fair, enrollments must be received in the office, and fees paid, by March 15, 2016.
  • NOTE—We NO LONGER take enrollment fees/forms for adult volunteers who have not taken the basic 5½-hour 4-H Leader Training. Once a leader has been trained and has passed OSU’s criminal background screening, we will accept their enrollment

LEADER ENROLLMENT 4-page form 

This 4-page (revised for 2015-16) form is the complete leader enrollment form that includes Health form, Volunteer Agreement, Code of Ethics, and Conditions of Volunteer (no other forms are needed for enrollment).
