4-H Enrollment Forms

Members must complete all of the following forms and pay their fees to enroll in 4-H:

• Member Enrollment Form 2012-13 (please contact your 4-H leader for forms)
• Official 4-H Code of Conduct Form
• Official 4-H Health Form

Members must pay $20 by January 31, 2013 ($25 after that date) in order to enroll in 4-H. $5 of that fee goes to the Wallowa County Leaders' Association and $15 goes to the State.  For more information on the State fee click here. 

Volunteers must complete the following forms:

• Volunteer Enrollment Form 2012-13 (please contact the Extension Office for form)
• Official 4-H Health Form
• Adult Volunteer 4-H Agreement
