2007-08 Enrollment Materials

All forms for volunteer mentors and members re-enrolling must be received at the Washington County 4-H office

by 5:00 p.m. on Friday December 14, 2007.

Enrollment Forms (make a copy of all forms for records)

• Member Enrollment Form **Volunteer Mentor, parent and member signatures are required on this form**
• Volunteer Mentor Enrollment Form, must be filled out by every mentor
• Health and Code of Conduct Form, must be filled out by every mentor and member

***Member fees are $11 per member, and $12 per horse member***

• 4-H Club/Group Annual Financial Statement/ Account Activity (must be turned in by December 14, 2007 whether or not you plan to enroll)

• State of Oregon Conditions of Volunteer Service and Insurance

All forms in the packet must be filled out completely.  Any incomplete forms will be returned to the Volunteer Mentor.  Publication orders will not be filled until all of the materials in the packet are completely filled out and received at the extension office.

Consequences for Late Enrollment

It is very important to meet the enrollment deadline.  If you miss the deadline, you must come into the Extension office to speak with Terry Palmer or Lisa Conroy to explain your late enrollment, as well as attend an additional training before you will be considered for re-enrollment.  This process needs to be completed quickly, for you will no longer be covered by insurance starting December 31, 2007.
