Do you have a plant, insect or other sample you would like
to submit to the OSU Yamhill County Master GardenersTM?

Here are some tips for filling out the “Intake Form” that will help the Master Gardeners answer your questions and contact you with results:

* AT THE TOP OF THE FORM, Please write your name, address and phone number clearly. This is how we will get in touch with you when we have results or to contact you if we need more information or a different type of sample. We will use your address to send any OSU reference publications related to your question (free of charge). We might also mail a follow-up questionnaire asking for an assessment of our services. If you
would like to be contacted by email, please write in your email address clearly. We preserve your privacy and do not release any of your contact information to any other organization.

* IN THE MIDDLE OF THE FORM, Please place an “X” by the type of inquiry you are submitting. If you have several different questions or samples, please fill out a separate form for each one to help avoid confusion.

* Give us as much information as you can about the question or sample. We will contact you if we need additional details or sample materials.

Your Samples

* PLANTS: More is better. Whenever possible, please supply large samples that include several examples of healthy and problem leaves, bark, flowers and/or fruits/cones. Placing samples in a sealed plastic bag will generally keep them in good shape.

* FLOWERS: Flowers and/or fruits are often the key to identifying “mystery” plants.If you know the plant will flower, please wait until you can bring some in for a positive identification.

* INSECTS: Generally, only one insect is necessary for a positive identification. The insect does not need to be alive, but it should be all in one piece. Insects can generally be safely trapped by placing the open mouth of a bottle over them, slipping a thin piece of cardboard or paper over the opening, then replacing with a lid. (You do not need to punch holes in the lid.)

* IMAGES: Sometimes photos or digital images can be very helpful with diagnosis and identifications. If you don’t have direct access to samples, try supplying images. You can send digital images relating to your questions via the form.


How to submit the Intake form and your samples

There are several ways you can submit your questions or information to the OSU Master GardenerTM volunteers:

If you have questions, but DO NOT have physical samples or photos to deliver to the Master Gardeners...

* Fill out our online form and submit it to the Master Gardeners with one click. You can attach digital images to the form as well. You can be linked to the online form by clicking on the daffodil at the bottom of this page.

* You can send an email to the Master Gardeners, using the Intake form information as a guide. You can attach digital images related to your questions to the email. The email address is Yamhillmg@oregonstate.edu

* You can call the Master Gardeners at 503-434-8918.

If you have physical samples to deliver to the Master Gardeners…

* Fill out the online form and submit it to the Master Gardeners with one click. Choose the option at the top that indicates you will be bringing in a sample. Link to the online form by clicking on the daffodil at the bottom of this page.

* Come to the Yamhill County Extension office and fill out an Intake Form while you are in the office. Any of the office staff can assist you.
