Filling out forms

Fillable PDF documents

  • Before entering anything, save the document to your desktop (or whatever folder you want). To do this, right click the name of the document and select "Save link as". A new window will appear where you can name your file and choose where to save it.
  • Open the file you just saved and begin entering your information. Be sure to save your work frequently.

Non-fillable PDF documents

  • Save the document to your desktop (or whatever folder you want). To do this, right click the name of the document and select "Save link as". A new window will appear where you can name your file and choose where to save it.
  • Print the document. The way to do this will depend on your computer setup.
  • Fill out the form by hand with a pen or pencil.

Microsoft Word documents

  • Save the document to your desktop (or whatever folder you want). To do this, right click the name of the document and select "Save link as". A new window will appear where you can name your file and choose where to save it.
  • You can type your information directly into the Word document. If you do this, DO NOT change the layout or formatting. Only work in the spaces intended for entering your information.
  • You can also print the document first and fill it in by hand. The way to print the document will depend on your computer setup.

Returning forms

Find your local 4-H program to find out how you need to return your form. There are a few ways this may be done:

  • Attaching the document to an email. This will require scanning any documents that were filled out by hand.
  • Returning the document to your club or local Extension office in person.
  • Mailing or faxing the document to your local Extension office.

Important note: For privacy and security, avoid sending forms over email that contain sensitive information, such as your date of birth or social security number.
