2025-2026 4-H Youth Enrollment Form

This 4-H Enrollment form is for Lake County members. Enrollment Fees are $40 general youth member (ages 9-19) and $35 for Cloverbud member (ages 5-8). Maximum family cost is $80. (Ages based on member's age as of September 1, 2024). Payments can be made by check (payable to Lake County 4-H Association), cash, or by credit card via our online store by going to https://beav.es/5o7. Please print, fill out completely, sign (both member and parent/guardian) and bring or mail to the Lake County Extension Office, 103 S E St, Lakeview by January 9, 2026, at 3pm. For questions call 541-947-6054. Please include all projects you plan to bring to fair. Animal projects are separated by breeding, market, and bottle. For goats, please list Dairy, Fiber, Pygmy, Meat, then include breeding, market, or bottle kid.

Lake County 4-H Ambassador Application

It is both an honor and a responsibility to be selected as a Lake County 4-H Ambassador. You are a representative of your community, Lake County 4-H, the Oregon 4-H program and a role model to all 4-H youth. This is a time commitment and leadership role. If you are interested in applying the following criteria apply: • Must be in grades 8-12 • Must be enrolled in the 4-H program with 2 years of 4-H experience. • Must be committed and available to serve the next 4-H year.
