General County Fair information and forms

2024 Fair Entry Form (in English): Due to the Crook County Extension Office the first Friday in July by 5:00 pm for all project except horse. Horse Fair entries due June 14th by 5:00 pm. All signatures required (parent, leader & member).
2024 Horse Declaration Form (in English): Due to the Extension Office by 5:00 pm on March 29, 2024
2024 Horse Fair Consent Form (in English): Due to the Crook County Extension Office June 14, 2024 by 5:00 pm
2024 Horse Stall Request Form (in English): Stall request/fee form for horse stalls. Due by June 14, 5:00 pm
2024 Livestock Pen Count Form (in English): Must be completed by leader, individual/family forms will not be accepted. Due Friday, July 5th by 5:00 pm at the Extension Office.

County Fair forms for market animal projects

Country of Origin (in English): Please fill out this Country of Origin form. One must be completed for each market animal at fair EXCEPT Beef and Swine.
Crook County 4-H Market Animal Health Certification (in English): Please sign this form if you are going to participate in the Crook County 4-H Fair with your market animal.

Oregon State Fair

Participating at the Oregon State 4-H Fair is a honor because 4-H youth have excelled with their projects at their local 4-H County Fairs and qualified to showcase their skills with other youth from across Oregon. Come experience the fun and celebrate their achievements.
