Fair Entry Forms

IT IS FAIR ENTRY TIME!

Every 4-H youth must complete and submit a 2025 4-H Exhibitor Packet, along with the individual project area entry forms.

[NEW FOR 2025] All Animal Science Participants are required to participate in the YQCA training and MUST submit a YQCA certificate with their fair entry paperwork on May 15th.

Fair Entry paperwork that is received without a YQCA certificate will not be accepted. Additional information about YQCA can be found here. (Cloverbuds and 4-H Exhibitors who are only participating in the Companion Pet Interviews do not need to take the YQCA Training.)

CHANGES FOR 2025:

This year, DOG and POULTRY Exhibitors have separate Entry Forms. The SMALL ANIMAL form remains the same for Rabbit, Cavy and Companion Pet exhibitors.

The CLOVERBUD Entry Form has changed! One side is for HEARTH Entries and the other side is for Small Animal Entries.

Any 4-H Members who would like assistance with Fair Entry Paperwork are invited to "drop in" to the 4-H Office from 6:00-7:30 p.m. on Wednesday, May 14th and 4-H Staff will be available to help with paperwork! The office is located at 1815 NW 169th Place, Suite 1000, Beaverton, Oregon 97006.

4-H Families are encouraged to contact Club Leaders or the 4-H Office with questions about filling out Fair Entry Paperwork. We are here to help answer any questions that you may have!
