Program Dollar Application (in English): Form for 4-H members who have Program Dollars that they want to use. Print out and submit completed form (including all signatures) to the Clackamas County Extension Office, attention CC4-HA Treasurer.

Annual Youth Safety Training instructions (in English): OSU requires all enrolled 4-H leaders to complete the Youth Safety Training each year. Follow these steps to access the training.

Members Residing Outside Clackamas County (in English): We must track all youth members whose primary residence is outside Clackamas County to comply with state 4-H rules.

4-H Member Cards (in English): Membership cards can be downloaded, printed, and distributed when members are enrolled.

Photo Opt Out Release (in English): This form is optional and only to be used if you DO NOT want us to use your/your child's photo or name in media. If you do submit this form, we will do our very best to not take pictures of you or your member and will not include your name in any press releases or stories.

4-H Club Annual Financial Statement (in English): DUE by August 1. Required for all clubs, even if you don't have a club treasury and don't do any fundraising. Not required for Independent groups.

4-H Club Materials and Equipment Inventory (in English): Keep an inventory of your 4-H club's materials, equipment, property and other assets by completing this form.

Club Assets Disbursement Form (in English): Any club that handles money or acquires material assets should have a clearly defined and known procedure or disposing of the funds or assets if the club should disband. Money or assets acquired in the name of 4-H should continue to be used for that purpose even if the original club disbands. The following options are recommended: a Clackamas County 4-H committee, the Oregon 4-H Foundation, or other approved charitable organization (MUST be approved by the County Extension agent). Disbursement among the members and leaders of the club is not acceptable. This form is to be turned in to the Extension office when a club that has assets disbands.

Fundraising and donation proposal worksheet (in English): A proposal is due TWO WEEKS before a planned activity to raise funds for a club through donations or fundraising. You must also submit a report form when your fundraiser is complete.

4-H Lending Library (in English): List of available books, binders, DVDs, learning kits, etc. that 4-H clubs and K-12 classrooms can borrow.

Energy Education Curriculum (in English): Renewable energy education lessons for science in school and after school.

Online Water and Environmental Education Resources (in English): These fun and educational resources are available free of charge to school teachers, parents, and volunteer 4-H leaders during this "stay at home time." Nature and natural resources exploration can be done safely without physical contact with others. Nature is everywhere from the spider visiting your home to your porch or backyard. Nature lends itself especially well to science education, mathematics, art, reading, and writing (poetry and journaling). Most activities require simple materials available at home.
