Record books

Completing a Record Book is a great way to learn more about your project and capture all the amazing things you've done during the year. By keeping records, you will also have an advantage when applying for awards, special trips, scholarships, and 4-H leadership positions.

State Awards Applications are due to our county office by 4pm on February 18. Apply now for State Scholarships, National Congress, State Ambassadors, and State Awards.

General record book forms and instructions

Metro 4-H Record Book Brochure (in English): Explains the pieces of a Record Book, how to put them all together, and the process for submitting your Record Book for Awards (due October 1).
4-H Memories: A how-to guide to filling out My 4-H Resume (in English): This guide takes you through each section of the My 4-H Resume, showing example entries, sharing tips for each section, and helping you decide what goes where.
Metro Record Book Application Page (in English): This is the first page that goes in your Record Book. It tells us a bit about you, your 4-H projects, and what awards you are qualified to receive (hint: check off lots of these).
4-H Advancement Certificate (in English): Have you completed an Advancement step this year? Print out an Advancement Certificate here!
Non-Competitive 4-H Record (in English): For Cloverbuds and older members who want to ease into record keeping.
Leadership Words (in English): Are you writing an application, resume, or essay? Using action verbs is a great way to explain what you did. Look through this list of Leadership Words to find a descriptive term that will make the reader take notice!

Civic Engagement Award (in English): The Civic Engagement Award will be presented to a maximum of two youth in each county who exhibit the qualities of dependability, citizenship/community service, leadership, and patriotism.
Youth Leadership Award (in English): The Youth Leadership Award recognizes up to two youth from each county who have demonstrated emerging leadership.

Project records and advancements

4-H Horse Fair Records Instructions and Advancement Grid (in English): Please use this guide to help you prepare your 4-H Record Book. Record Books are required to participate in the Clackamas County 4-H Horse fair.
4-H Youth Leadership Advancement Program (in English): Use this Advancement to track the skills you learn in your Leadership projects. Advancement forms are encouraged but not required in Record Books turned in for awards.
4-H Youth Leadership Record Form (in English): Use this record for any youth leader project, including Ambassadors, Camp Counselor, Junior Leader, Teen Leader, and Junior Superintendent. Combine all your 4-H Leadership projects on one form.
Clackamas County 4-H Horse Record Check Sheet (in English): Please fill out this and include it inside the front cover of your Record Book to ensure your Records are reviewed FOR HORSE FAIR QUALIFICATION ONLY.
