Financial Assistance Request Form: The Jackson County 4-H Association is committed to having no financial barriers between youth and the 4-H Youth Development Program. This application is for members needing financial assistance for enrollment or other 4-H activities.
Club Enrollment Fee Worksheet: For clubs which pay their member's enrollment fees out of the club treasury. Leaders should take this form with payment to the Extension Office.
Youth Enrollment Form: This form should not be used for enrollment, but is included for informational purposes only as it includes 4-H policies. Please enroll online at the orange link above.
Volunteer Enrollment
Returning Volunteer Enrollment Site: This is where volunteers must complete their enrollment each year between October 1st-December 31st. New or prospective volunteers should not enroll here until instructed by 4-H staff.
Annual Club Registration Form: Paper form. Used by main leader to re-register the club for the upcoming 4-H year. Due to the Extension office September 1. The other, online version is preferred.

New Volunteer Interest Form: The first step to getting involved with 4-H for any adults who want to volunteer is to fill out this form! Once you fill it out, we'll get back to you shortly.
New Volunteer Instructions: This welcome letter outlines step-by-step instructions for new volunteers to begin their journey with 4-H!
New Volunteer Application Form: This form must be fully filled out by all prospective volunteers and submitted as soon as possible to get the process started.
Club Financial Peer Review Form: This form should be completed each year and filed with the club/unit copy of the annual financial summary. The Peer Review Committee may set up a timetable for any corrections that need to be made and review the books for compliance on that date.

Jackson County Animal Science Record: Needed for fair and record books. This is for Jackson County only. We will not use the state animal science record at this time.
My 4-H Resume: This record is your entire 4-H career in one document. Keep and update for your entire 4-H career.

Market Animal Code of Ethics Form: Please complete this form if you are entering any market animal in the Fair and bring to fair with you.
Sheep Declaration Form: Please complete this form and turn it in at the same time you get your market lambs tagged (1st Saturday in March). Only breeding sheep need to be recorded on this form.
