2026 Fair Entry Form: Due to the Crook County Extension Office Tuesday, June 30th by 5:00 pm. All signatures required (parent, leader & member).
2026 Horse Fair Entry Form: Due to the Crook County Extension Office Tuesday, June 30th by 5:00 pm. All signatures required (parent, leader & member).
2026 Horse Stall Request Form: Stall request/Exhibitor fee form for Horse Fair. Must be completed by leader, individual/family forms will not be accepted. Due Tuesday, June 30th by 5:00 pm at the Extension Office.

Country of Origin: Please fill out this Country of Origin form. One must be completed for each market animal at fair EXCEPT Beef and Swine.
Crook County 4-H Market Animal Health Certification: Please sign this form if you are going to participate in the Crook County 4-H Fair with your market animal.
Buyer Photo Specifications: Photos MUST be received by Friday, July 24, 2026 at 5:00 pm
