Clackamas County 4-H Teen Leader Application: Teens! Are you interested in working with an adult advisor to lead your own club? Complete the Teen Leader Application. Adult Leader Enrollment Packet: ALL NEW 4-H Adult Volunteers must complete and turn a paper enrollment packet after they have been approved as a volunteer. Youth Conditions of Volunteer Service form: Any member in a youth volunteer role must complete and turn in this form. Includes Junior Leader, Teen Leader, 4-H Ambassador, Camp Counselor and Junior Superintendent.
