4-H Club Charter: To apply for a club charter, complete and submit this form to your 4-H staff at your local Extension office.
4-H Annual Report for group exemption status to Oregon 4-H Foundation: As a member under the IRS Group Exemption #5946, the following information is needed for the Annual Report (due no later than January 31 of each year for the fiscal year ending the previous June 30
4-H Annual Financial Statement for Clubs and Committees: All 4-H clubs and 4-H committees must complete this form even if you have no income and/or are not planning to have a group for the upcoming 4-H year. Contact your local county 4-H program / Extension office if your group is disbanding this year. (Report income/expenses from July 1 to June 30)
Banking and Financial Agreement: Please read over and sign this document in order to comply with non-profit, charitable corporation rules when opening and maintaining a bank account for your 4-H club or committee.
4-H Club Materials and Equipment Inventory: Please use this form to keep track of the materials, equipment and other assets your 4-H club or committee acquires.
4-H Club or Committee Fundraising Worksheet: Please use this worksheet if your 4-H club or committee is planning a fundraising event.
Request to Conduct 4-H Fundraiser: Please use this form to submit a request if you are planning a fundraiser for your 4-H club or committee. Your request must be approved prior to advertising, promoting, or holding the event.
Financial Gift or Donation: Please use this receipt template when your 4-H club or association receives a financial gift or donation.
Authorization to Open a Bank Account: Use this letter to open a bank account on behalf of your club or association.
